NAVIGATE THE
VA SYSTEM
We will help you understand the VA benefits you may be entitled to, enroll/register veterans into the VA, apply for medical care and benefits, and schedule and provide transportation to VA appointments.
PROVIDE FAMILY-LIKE SUPPORT
We will be a family to veterans who no longer have one, escort veterans to the VA, be by their side, and ensure they ask the right questions of their Primary Care Physician.
ENSURE A SAFE ENVIRONMENT
Help find suitable, affordable, and safe housing and explore care options with their home.
ASSIST WITH SELF-CARE AND MEDICAL EQUIPMENT
Meet various self-care needs and coordinate additional medical equipment, such as wheelchairs or walkers.

PRE-NEED FOR BURIAL IN VA CEMETERY
Pre-Need means before death. Only complete this form if you are applying for a Pre-Need determination of eligibility for burial in a VA national cemetery.
If the individual is already deceased, contact a local funeral home or the National Cemetery Scheduling Office at 1-800-535-1117 to expedite processing.
VA ENROLLMENT
VA form 1010EZ is for enrollment into the VA for veterans to apply for enrollment in the VA health care system. The information provided on this form will be used by VA to determine eligibility for medical benefits.
MILITARY SERVICE RECORD
Standard form 180 is used to request military records from the National Personnel Records Center. The DD Form 214 was issued to veterans who served between 1950 to the present. Veterans serving between 1941-1949 were issued either "WD AGO" (War Department Adjutant General's Office) forms or the NAVPERS.
